“The health department believes very strongly that you should learn the value of giving back to your community through volunteer activities.   The rewards far outweigh the effort.”

 

 

You must complete 10 hours of community service.  This is a required part of this course and health is a high school graduation requirement.  It MUST be completed to earn credit!

 

Requirements For Project

 

1.  All hours must be health related and/or helping the community.

 

1. All volunteer work must be approved in advance.  No credit will be given for unapproved work.

1. All hours need to be completed through an agency.  (Helping out a neighbor is not an approved activity.)

​

4.  All hours must be completed within the time frame of this semester.

 

5.  All hours must be documented the following way:

1. on professional stationary

2. signatures of people you worked for

3. dates of service

4. hours of service

5. description of service

6. phone number of agency

 

6.  All hours must not be underneath the advisement of a relative.

 

7. All hours must be turned in by the final deadline to receive credit.

 

Early Deadline: MARCH 11, 2016 = +25 AND APRIL 21, 2016 = +15

 

Final Deadline:  MAY 19, 2016

 

Please start EARLY and be RESPONSIBLE!  NO extensions will be granted!
